Case Management
We provide case management services to the vulnerable and underserved communities on the islands of Oahu, Maui, and Kauai, as well as in Hilo. The populations include but are not limited to justice-impacted individuals, houseless, drug-using community, and those at risk of houselessness.
Our primary focus through case management is quality of life improvements for the participants we serve utilizing Harm Reduction and person-first principles of service.
What are harm reduction principles and Principles of person first care?
What are harm reduction principles? Harm reduction refers to interventions that are aimed at the reduction of negative effects of health and quality of life without necessarily ceasing the problematic health behavior.
What are the principles of person first care? The four primarily principles of person centered service are dignity, compassion, respect, coordination and personalization of services.
These services include but are not limited to: pre and post tenancy support, obtaining MedQuest and SNAP benefits, employment support and navigation, referrals to community substance use providers, coordination and referrals to community housing partners, engaging our participants with primary care providers in the community, and supporting our participants in gaining life skills necessary to ensure long term improvement to their quality of life.
